Elite Residential Habilitation welcomes the opportunity to serve new program participants. We provide guidance and support to help make sure all of the requirements are in place for individuals to receive support from our dedicated team.

Eligibility Requirements
and Approval
A set of eligibility requirements must be met before support services are provided. To receive support services from Elite Residential Habilitation, an individual must:

Have Idaho Medicaid approval for residential habilitation

Have a developmental disability diagnosis

Be on the Medicaid Developmental Disabilities (DD) Waiver or the Medicaid Aged & Disabilities (A&D) Waiver

Have a current plan of care or be working with a case manager

Require support for daily living, safety, or skill development

Have an active SIBR (Scales of Independent Behavior-Revised) assessment through Liberty Healthcare

Have Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI) coverage
Medicaid Approval
A requirement for participation in residential habilitation is Medicaid approval of identified services.
The process for this approval is facilitated by a case manager or targeted service coordinator.
During this process, a needs assessment is completed, followed by the creation of a care plan that outlines a potential participant’s goals and required support hours.
If services are approved through Medicaid, we will move forward with participant onboarding
and placement.
Starting Services
At the onset of services, a meet‑and‑greet is held with program participants and members of the Elite leadership team.
In cases of residential habilitation services, a home tour and compatibility
review are provided. Individualized support plans are developed, and individuals
are introduced to staff members and program routines. We also offer those in
the residential habilitation program support during the transition into
designated residences.
