Elite Residential Habilitation provides adult disability services in Idaho Falls. We welcome individuals into the Elite community, offering support to foster independence and more integrated living. 

The services we provide are backed by our goal of building futures and empowering lives. We operate under a transparent, benefit model in which payment is tied directly to the services provided and the hours authorized by Medicaid. Focus is on clear accountability, accurate billing, no hidden costs, and direct alignment between services delivered and services billed.

Elite services are available to individuals 18 and older with the required approval. Please contact our team for assistance with

Our team offers personalized support while respecting the choices and rights of program participants. We provide 24-hour residential habilitation services, hourly supported living services, and other service offerings.

Residential Habilitation Services

(24-Hour Support)

We provide high- or intense-level of care through our 24-hour residential habilitation support.

The purpose of 24-hour services is to provide participants with safe environments and opportunities to live productive lives. Our objective is to provide residences that are a more independent-oriented alternative to nursing homes or other long-term care facilities.

Our well-maintained homes are located in safe residential neighborhoods. Each home is designed to feel warm, comfortable, and truly lived‑in, not clinical or institutional. The homes include fully furnished living spaces, shared common areas, and private bedrooms. Also, our homes have accessible layouts with safety features appropriate for participant needs and on-site staff available based on
individual support plans.

Housing placement considerations include compatibility with other residents, level of support required, gender and safety considerations, and, when possible, personal preferences. 

Most homes have 1-3 residents, and we strive to provide a balance of independence, social interaction, and individualized support. Individuals may remain in an Elite home as long as it aligns with their needs, goals, and Medicaid authorization. Participants may stay long‑term, until their needs change, or until they choose different living arrangements. The main goal is stability, not temporary placement. 

 

High-Level Support

High-level support is for individuals at a higher behavior management level than those who qualify for intense support and who do not have intense medical requirements. This type of service provides supervision of individuals at a ratio of three participants to one direct care provider.

Intense Support

This service is typically provided for individuals whose medical needs are severe or who display aggressiveness. Intense support is provided with a ratio of one participant to one direct care provider and involves supervision 24 hours per day, every day.

What Residential Habilitation
Does Not Include

  • Medical or nursing services

  • Mental health therapy or counseling

  • Employment services

  • Transportation unrelated to habilitative goals

  • Room and board (unless part of a specific program structure)

Services provided in hospitals, institutions, or skilled nursing facilities are not included in our services.

Hourly Supported Living Services

Our hourly supported living services provide lower levels of support than our 24-hour services. The purpose of hourly services is to assist participants in becoming more independent, learning new skills, and integrating into the community. 

An important aspect of hourly services is providing instruction and prompts in a positive atmosphere to help participants strive to reach their goals in all areas of their lives. 

With hourly supported living services, participants are provided direct care from our staff, usually for 30-70 hours per week, based on participant need and preference.

Staff members primarily work with participants to help them improve independence in areas such as home maintenance, hygiene, meal preparation, self-advocacy, and more.

Elite Residential Habilitation offers a range of support services.

Money Management
(Payee Services)

We offer services to help participants track and budget their money. Participants may choose Elite as their payee, and we will help ensure bills are paid on time and money is kept safe. 

As part of this service, weekly meetings are held for participants to review their budgets, pay bills, and receive money for food, activities, etc.

Employment
Support


Services include employment support. We offer help in searching for employment, applying for jobs, and preparing for job interviews.

Benefit
Collection


Our services include helping participants secure Supplemental Security Income and Supplemental Nutrition Assistance Program (SNAP) benefits, if they qualify.

Weekly and
Monthly Activities


Hosting activities is an important aspect of our services. We hold small weekly activities and also larger monthly parties.

 

 

Weekly activities may include:

  • Bowling

  • Movie night

  • Cooking classes

  • Craft sessions

  • Fishing / Outdoor activities 

  • Walks in the community

  • Shopping outings

  • Activities at Elite’s activity center or theater room

Monthly events may include:

  • Holiday‑themed parties

  • BBQs or outdoor gatherings

  • Community dances

  • Talent shows

  • Birthday celebrations

  • Group outings (zoo, museum, sporting events)

Transportation
Services

 

We provide transportation services to help participants attend appointments. Talk to our team about transportation availability and options.

 

BehaviorManagement
Support


Helping with behavior management is a priority for Elite. We use evidence-based approaches to behavior management while applying personalization to support program participants.

Education for Family
and Friends

Educational support is available to family and friends of Elite participants, including help identifying resources and more.
